What this permission means
This optional form lets a parent or legal guardian approve all, some, or none of five separate uses of classroom recordings. Your choice will not affect lessons, booking, fees, enrollment, or treatment.
Only purposes marked Approved may be used. Mr. Bob normally crops, blurs, or otherwise protects the student's identity. Approved permissions last while the student studies with Mr. Bob and for five years after the final class, unless withdrawn earlier.

1. Voluntary choice
This form records the decision of the student's parent or legal guardian for Robert Louis Pruss, professionally known as Mr. Bob, and Mr. Bob's ESL Class in Chiang Mai, Thailand.
Every permission is optional. Refusing all permissions or any individual permission will not affect lessons, booking, scheduling, fees, enrollment, or treatment.
2. Separate permissions
The guardian may approve all, some, or none of the five permissions. Agree to All approves all five. Deny All declines all five. A use is allowed only when its permission is marked Approved in the most recent decision sent to Mr. Bob.
Private review does not authorize sharing. Private sharing does not authorize public posting. Public website or social-media use does not authorize paid advertising. Guardian authority and electronic signature are separate confirmations.
3. Privacy protections and respectful use
Mr. Bob will normally conceal, crop, blur, or minimize the student's identifiable facial image. A student's voice, first name, answers, classroom participation, or written work may appear only when reasonably necessary for an approved use.
Mr. Bob will not intentionally publish the student's full legal name, school, home address, contact accounts, passwords, meeting details, private messages, sensitive family information, or formal academic records.
Without separate express permission, material will not be used for AI voice or face cloning, deepfakes, biometric identification, or general AI-model training, and will not knowingly be edited in a humiliating, deceptive, defamatory, or materially misleading way.
4. Authorized editors or service providers
When approved, necessary access may be given to contracted editors, hosting or technical providers, advertising platforms, or similar providers, including processing outside mainland China. Access must be limited to work needed for another approved purpose.
If this permission is declined, Mr. Bob must not give the recording or excerpt to an outside provider under this decision. The form never changes the guardian's selections automatically.
5. Duration and withdrawal
Each Approved permission remains active while the student studies with Mr. Bob and for five years after the student's final class, unless withdrawn earlier. A Declined permission never becomes Approved merely through the passage of time.
A parent or legal guardian may withdraw all permissions or request a narrower change at any time by emailing mrbobseslclass@gmail.com or messaging WeChat MrBob0725. After a verified withdrawal, Mr. Bob will stop new use covered by the withdrawn permission and take reasonable steps concerning material under his control.
Withdrawal does not undo authorized use completed before withdrawal and cannot guarantee removal of copies independently downloaded, cached, archived, printed, shared, or republished by third parties outside Mr. Bob's control.
6. Electronic signature and emailed decision
Typing the guardian's full name, confirming guardian authority, confirming the selections, and submitting the form constitutes the guardian's electronic signature. The completed decision is emailed directly to Mr. Bob with every permission, the form language and version, and the submission date and Thailand time.
This simple form does not create a website account, database record, or stored PDF. A parent who is uncertain about the effect of this decision should obtain independent legal advice before submitting it.
